So here is the research.
Expressive writing and reflective journaling are not wellness trends. They are interventions with a documented evidence base that spans decades and includes studies focused specifically on nurses and healthcare workers. The findings are consistent enough to take seriously.
What Expressive Writing Actually Is
Expressive writing, first studied systematically by psychologist James Pennebaker in the 1980s, involves writing about thoughts and feelings surrounding stressful or emotional experiences, rather than simply describing events. The distinction matters. Narrative writing that processes emotion produces different results than writing that simply recounts facts.
In study after study, participants who engaged in structured expressive writing showed improvements in psychological well-being, reduced symptoms of anxiety and depression, better immune function, and lower self-reported stress. The effects are not dramatic after a single session, but they are reliable across populations and cumulative over time.

What the Research Shows for Nurses Specifically
The application of expressive writing research to nursing and healthcare is relatively recent, but the findings are meaningful.
A 2021 study published in Frontiers in Psychology examined expressive writing among healthcare workers during the COVID-19 pandemic and found that those who engaged in structured emotional writing showed improvements in adaptive coping strategies and communication satisfaction at work. The study noted that expressive writing was particularly useful for nurses in high-stress environments.
A 2022 Doctor of Nursing Practice project [PDF] from the University of Nebraska Medical Center examined reflective journaling as an intervention specifically for nurse burnout. The study found that participants reported the journals as a meaningful outlet after shifts, with improvements in self-awareness and professional well-being. Nurses described wishing the journaling intervention would continue after the study ended.
A 2023 systematic review published in a peer-reviewed occupational health journal reviewed 13 studies on expressive writing as a workplace stress intervention and found that 9 of those studies showed significant positive effects on mental health variables including depressive symptoms, personal resources such as self-efficacy, and organizational variables including job satisfaction.
Why Writing Works: What the Science Suggests
Researchers have proposed several mechanisms to explain why writing about emotional experiences produces measurable effects.
It activates meaning-making
Writing requires the brain to organize experience into language and sequence. That process of organizing and narrating activates the prefrontal cortex, which is involved in rational processing and emotional regulation. Putting a difficult experience into words essentially helps the brain categorize and store it, reducing its ongoing emotional weight.
It creates distance from the experience
There is a well-documented phenomenon in psychology called self-distancing, the ability to observe your own emotional experience rather than being fully immersed in it. Writing in the third person or using structured prompts to reflect on an experience has been shown to facilitate this kind of distancing, making difficult emotions easier to examine and process.
It breaks the cycle of rumination
Unprocessed stress tends to recycle. The mind returns to it during quiet moments, disrupting sleep, focus, and rest. Writing gives rumination a destination. It moves the experience from passive replay to active processing, which reduces the frequency and intensity with which the brain returns to it.
What This Means for Nurses
Nurses already understand the value of evidence-based practice. The evidence for writing as a burnout and stress intervention is solid enough to meet that standard.
It does not require a therapist, a program, or a significant time investment. It requires a few minutes, a surface to write on, and a willingness to be honest about what the shift actually felt like.
That is a low bar for an intervention with a documented track record. For nurses who are skeptical of wellness recommendations that feel soft or unscientific, the research is worth reading.
