How Freelance Health Writing Actually Works

Most articles about freelance health writing answer the wrong question. They tell you what it “is” in theoretical terms: it’s self-employment; it’s writing health articles. But they don't tell you how it works as a business.

Those are different questions. The first one gets you interested. The second one gets you paid.

After 26 years inside this industry, I want to show you how the business actually works.

Where the work comes from

I get paid by two kinds of buyers: publications and companies. The work, the pay, and the path to landing it look different for each.

Publications are magazines, newspapers, online outlets, trade publications. They publish news, feature articles, profiles, and so on.

Companies are everything else. Hospitals, health tech firms, healthcare communications agencies, marketing agencies, employee benefits platforms, telehealth services. They publish content for marketing, education, internal communications.

When I started as a writer, journalism was a strong on-ramp for freelancers. Today, I'd point most newcomers toward content marketing instead. The volume of work is higher. The rates are stronger. The path to a sustainable income is shorter.

You approach publications with a query. For companies, you send Letters of Introduction (LOIs). Both kinds of clients hire freelance writers all the time. The best opportunities never get announced on job boards.

How the money actually moves

Three pricing models cover most of the income I've earned and seen earned.

Per-project. You quote a flat fee for the deliverable, the client agrees, the work happens, you invoice. This is how I've priced my work for years. It's the dominant model in content marketing.

Per-word. Common in journalism. The publication pays a per-word rate (often somewhere between 50 cents and $2 per word) on a defined word count.

Project retainer. The client pays a fixed monthly fee for an agreed scope of ongoing work. I've had retainer clients pay $1,000 per month for two short blog posts and $4,500 per month on annual contracts for eight short health articles. This is where the math gets really good once you've been at it a while.

Per-hour pricing exists. I avoid it. Hourly billing penalizes you for getting faster at the work, and you will get faster.

How the work flows

A typical freelance health writing project moves through five stages, give or take.

Outreach. I send an LOI to a company, or a query to a publication. Many don't respond. Some do.

Scoping. The interested client describes what they want. I ask the questions to nail down specifics.

Quoting. I name a price. The client agrees, negotiates, or moves on.

Doing the work. Research, drafting, revisions, delivery.

Getting paid. I invoice, the client pays, I follow up if the payment is late. Sometimes I pitch a follow-on project at the same time.

Multiply this five-stage flow by however many active clients you have, and that's a freelance health writing business.

What the income actually looks like

Income for freelance health writers ranges widely, mostly because we choose how much we want to work. One writer might take on 10 projects a month. Another might want two. Both can call themselves successful, because for many writers the goal isn't a particular income — it's more free time. You get to decide what success means for you.

In your first year, working part-time around a clinical job, you might earn $8,000 to $12,000. That's not a replacement income yet, but it’s excellent side-gig money.

By year three or four, working roughly 25 to 30 hours a week, $60,000 to $120,000 is in range.

Above that range is real, too, and not unusual. My own freelance business grossed around $146,000 in 2020, and I only worked roughly 24 hours a week. Six-figure freelance health writing income is a working reality for writers who have built strong client relationships, work with retainer clients, and have moved into higher-value deliverables like white papers and case studies.

None of these numbers come fast. The first year is the hardest. The math gets better every year after.

Why this business favors nurses

Here’s what editors have told me: they want writers who already understand healthcare. Many freelance writers don't. The ones who do command higher rates and get repeat work.

A nurse with one year of clinical experience already knows what other health writers spend years learning. The vocabulary. The workflow of a clinical visit. The dynamics between providers and patients. What staff and management really care about. The pain points on the patient side and the employee/employer side.

That knowledge is your primary asset. It is what clients are paying you for, not just your writing skills. The writing skill matters. The clinical knowledge is what differentiates.

Which is why this business works, and why it works specifically well for nurses.
