If you are new to freelance health writing, one of the first questions you will wrestle with is how to charge for your work. Charge by the hour? By the word? By the project? It sounds like a logistics question, but it is actually a values question. How you price your services shapes how clients perceive you, how much you earn over time, and whether your business feels sustainable or exhausting.

Most experienced freelance health writers price by the project. There are good reasons for that, and understanding them will save you from the most common pricing mistake new writers make.


The Problem with Hourly Rates

Hourly pricing seems fair on the surface. You work, you get paid. Simple. But it contains a trap: it punishes you for getting better at your job.

When you are new to health writing, a 1,000-word article might take you four hours. As you gain experience, that same article might take two. If you are charging by the hour, your income drops as your skills improve. That is exactly backwards. Expertise should be rewarded, not penalized.

Hourly pricing also creates friction with clients. Every invoice becomes a negotiation. Every hour logged is a potential point of dispute. Clients start watching the clock instead of evaluating the quality of your work. That dynamic does not serve anyone well.


What Project Pricing Does Instead

When you price by the project, you are pricing the deliverable, not the process. The client knows upfront what they will pay, and you know upfront what you will earn. There are no surprises on either side.

This matters especially in health writing, where the value of the content is rarely about how long it took to produce. A well-researched, clinically accurate blog post for a healthcare brand is worth what it is worth because of what it is, not because of how many hours went into it. A client who hires you to write that post is paying for accuracy, authority, and a finished product they can publish with confidence. None of those things are measured in hours.

Project pricing also gives you room to factor in everything that goes into a piece: research time, revision rounds, source verification, and the background knowledge you bring to the work. Nurses, in particular, carry a reservoir of clinical understanding that dramatically reduces the time they need to get up to speed on a health topic. That knowledge has value. Project pricing lets you build that value into your rate without having to justify it line by line.

The Signal Your Pricing Sends

How you price is also a signal to prospective clients about how you operate. Writers who charge by the hour often position themselves as vendors. Writers who charge by the project position themselves as professionals. Clients who are accustomed to working with skilled freelancers generally expect project pricing. It is the norm in content marketing and health journalism at the professional level.

That does not mean you should never discuss your rates in hourly terms. Some clients will ask what your "hourly equivalent" is, especially if they are comparing you to other options. Knowing that number helps you have that conversation. But leading with a project rate tells the client that you know what your work is worth and that you are confident in delivering it.


Pricing Is a Skill You Develop

New health writers often underprice out of uncertainty. They are not sure what the market will bear, and they are not sure they can defend a higher number. That uncertainty is normal. But the answer is not to default to hourly pricing because it feels safer. The answer is to start building your understanding of the market, talking to other writers, and developing the professional confidence that comes from delivering good work consistently.

Project pricing requires you to estimate your time, know your market, and stand behind your rates. It asks more of you than tracking hours does. But it also gives you more: more earning potential, more professional clarity, and a better foundation for a freelance business that actually grows.

Freelance health writing is a skill. Clinical expertise is an asset. Project pricing is how you make sure the market knows it.
